Specialized Neurologic and Complex Rehabilitation Clinic
The Specialized Neurologic and Complex Rehabilitation Clinic provides a specialized and comprehensive rehabilitation service for patients with complex medical and physical impairments with the aim of improving mobility and overall physical function. Using movement as medicine, this interprofessional team, consists of Physiotherapists, Occupational Therapists, Speech Language Pathologists and Certified Rehabilitation Assistants. Together, they collaboratively offer a comprehensive and customized patient program based on best practice, while educating the patient and family with the knowledge and skills to manage their own health.
Services are provided to discharged inpatient rehabilitation patients, appropriate acute care and community patients in a timely manner to ensure that the flow of patients through the rehabilitation continuum remains fluid.
Goals
Outpatient Physiotherapy and Occupational Therapy Services
To provide condition specific treatment for patients with complex medical and physical conditions who experience functional deficits of various origins including those caused by strokes and neuro-degenerative diseases.
Outpatient Speech Language Pathology Services
To provide speech services and education to individuals with speech and swallowing difficulty, so that they are aware of the etiology of their deficits and typical course of treatment.
Hands Clinic
To provide evidence based hand and upper extremity therapy as well as splinting and orthotic services.
Referrals
Referrals for all Outpatient Clinic Services must be made by a physician or nurse practitioner.
Program Criteria for Outpatient Physiotherapy, Occupational Therapy and Speech Language Pathology Services
The patient must:
- have treatment needs that cannot be met by community services;
- have potential for functional and/or neurological improvement;
- be medically stable and have significant endurance to participate in therapies;
- have achievable rehabilitation goals;
- be willing to participate in the program;
- be able to benefit from a goal-oriented, time-limited program.
Program Exclusions for Outpatient Physiotherapy, Occupational Therapy and Speech Language Pathology Services
The patient is not eligible to participate in the program if they are:
- eligible to receive similar services (PT/OT/SLP) already in existence within the community;
- eligible to receive other similar services (PT/OT/SLP) funded through auto-insurance, the Workplace Safety Insurance Board (WSIB) or other government funded healthcare programs (example: in-home physiotherapy);
- looking to keep an existing level of function.
Cost
Specialized Neurologic and Complex Rehabilitation Clinic services at HDGH are funded by the Ministry of Health. Services are provided without direct costs upon presentation of a valid OHIP card. There will be a cost affiliated if referred to the Hand Clinic for splinting.
