Reporting to the Integrated Manager of Outpatient Mental Health Services, the Social Worker II will be responsible for the delivery of Social Work to treat individuals with diagnosed with Mood & Anxiety (mental illness) and concurrent disorders. The Social Worker II will provide direct patient services including assessments, counseling, treatment groups, consultation with program team and community partners, and referrals to appropriate organizations/professionals. The Social Worker II is responsible for delivery of care consistent with the goals and objectives of the hospital and the standards of relevant professional organizations. The Social Worker II complies with the Occupational Health and Safety standards for the hospital and is proactive in promoting a safe work environment.
Social Work knowledge, skills and experience appropriate to the target population (e.g. Bio psychosocial assessments, goal oriented treatment planning, counseling and treatment, strong interviewing and relationship building skills, organizational and evaluation skills) - Required
Experience working with individuals struggling with addictions – Required
Demonstrated competency in Dialectical Behaviour Therapy, Acceptance and Commitment Therapy, And Cognitive Behaviour Therapy modalities – 1 year experience – Required
Relevant treatment group facilitation experience and interest - Preferred
Excellent verbal and written communication skills
Good problem solving skills and ability to think analytically, innovatively and independently
Ability to work effectively as a member of a multidisciplinary health care team
An interest in working with clients who struggle with serious mental illness (SMI) and/or addictions
Strong understanding of community addictions support networks and referral practices/ processes with relevant community partners
Commitment to and participation in research activities/ program projects
Strong understanding and commitment to social work standards of practice, documentation requirements and code of ethics
French Language proficiency is an asset
Education and/or Certification:
Master of Social Work Degree – Required
Relevant internship/placement educational experience, or work-related experience - Preferred
Current Certification of Registration in good standing with the Ontario College of Social Worker and Social Service Workers – Required
DBT Certification/Training – Required
